fa مقایسه اثر زنجبیل ، گاباپنتین و اندانسترون در پیشگیری از تهوع و استفراغ بعد از سزارین به روش اسپاینال Comparison of the effect of Ginger, Gabapentin , and ondansetron for prevention of nausea and vomiting after cesarean section by spinal anesthesia پژوهشي Research مقدمه: تهوع و استفراغ، از شایع‌ترین عوارض عمل جراحی سزارین هستند که ممکن است باعث پارگی بخیه، پارگی مری، آسپیراسیون معده و طولانی شدن مدت بستری شوند. با توجه به اهمیت موضوع و ضرورت یافتن راهی برای پیشگیری از تهوع و استفراغ، این مطالعه به مقایسه‌ی اثر زنجبیل، گاباپنتین و اندانسترون در پیشگیری از تهوع و استفراغ بعد از سزارین به روش اسپاینال می‌پردازد.<o:p /> مواد و روش‌ها: این مطالعه یک کارآزمایی بالینی دو سو کور تصادفی بود که بر روی 208 زن کاندیدای سزارین انتخابی انجام گردید. پس از اخذ رضایت‌نامه‌ی کتبی، نمونه‌ها به چهار گروه مساوی تقسیم شدند. به گروه اول 300 میلی‌گرم گاباپنتین، به گروه دوم 8 میلی‌گرم اندانسترون، به گروه سوم یک گرم زنجبیل و به گروه چهارم دارونما داده شد. قبل از انجام اسپاینال و بعد از عمل در ریکاوری و نیز در فواصل زمانی 2، 4 و 6 ساعت بعد از عمل، شدت تهوع و استفراغ نمونهها بر اساس مقیاس چهارتایی ثبت گردید. داده‌ها در نرم‌افزار SPSS نسخه‌ی 19 و با استفاده از آزمون‌های آنالیز واریانس یک‌طرفه، تی مستقل و کای دو مورد تجزیه‌وتحلیل قرار گرفت.<o:p /> یافته ها: بین چهار گروه مذکور اختلاف معنی‌داری از نظر سن، تعداد بارداری، میانگین فشار خون و ضربان قلب مادر، قبل و حین عمل و آپگار نوزادان و سطح هوشیاری مادران وجود نداشت. میانگین تهوع و استفراغ در گروه گاباپنتین (0/34± 0/28)، گروه اندانسترون (0/25± 0/28) و گروه زنجبیل (0/85± 0/27) بود که به‌طور معنی‌داری کمتر از دارونما (0/22± 1) بود (p=0/01)؛ ولی اختلاف معنی‌داری بین سه گروه دیده نشد. نتیجه‌گیری: می‌توان از گاباپنتین، اندانسترون و زنجبیل با اثرات مشابه در کنترل تهوع و استفراغ بعد از عمل در خانم‌های کاندید سزارین استفاده کرد.<o:p /> Introduction: Nausea and vomiting are among the most common complications of Caesarean section and may cause suture dehiscence, esophageal perforation, gastric aspiration, and delay in hospital discharge. The aim of this study was to compare the antiemetic effect of Ginger, Gabapentin, and Ondansetron for prevention of nausea and vomiting after cesarean section by spinal anesthesia.<o:p /> Methods: This randomized, double-blind placebo controlled clinical trial was done on 208 women undergoing elective cesarean section. After taking written informed consents, they were divided into four equal groups. 300 mg Gabapentin, 8 mg Ondansetron, 1 gr Ginger, and placebo were administered to each group. Before performing spinal anesthesia and after the operation in recovery, and also in 2, 4, and 6 hours intervals after the surgery, nausea and vomiting were recorded based on a four point scale. Data was analyzed by SPSS 19 using one way ANOVA, independent t-test and Chi square.<o:p /> Results: There were no significant differences among the four groups according to age, gravidity mothers` mean atrial pressure and heart rate before and during surgery, newborns` APGAR score and mothers` consciousness. Nausea and vomiting in Gabapentin (0.28±0.34), Ondansetron (0.28±0.25), and Ginger (0.27±0.85) groups were significantly less than placebo group (1±0.22) (p=0.01), whereas there were no significant differences among the three groups (p=0.05).<o:p /> Conclusion: Using Gabapentin, Ondansetron and Ginger with the same antiemetic effect are appropriate for women undergoing cesarean section.<o:p /> سزارین, بی‌حسی اسپاینال, تهوع, استفراغ, گاباپنتین, زنجبیل, اندانسترون.
